Based on national polling, claims data, and its own network, the report reveals that the need for help continues to grow while access, affordability, and clarity on how to get there still fall short. For many, mental health care is a way of labyrinth feeding into a “maze of dead-ends”.

The State of Mental Health Report 2026 by Rula reveals interconnected three developments: increased acceptance of seeking help, increased acceptance of new delivery approaches, and still barriers for millions due to provider shortages, insurance issues, and the confusing system structure. The “modern mental health maze” comprises waiting lists, out-of-network shockers, and disconnected services that make continuity impossible. Patients jump from primary care to ED to different providers.

Younger adults and parents are also most likely to call out that they experience worry and stress, and that they are stuck on what step to take first. Although stigma has decreased, many still feel inundated by an endless crowd of apps, platforms, providers, and programs with their various charges, degrees of quality, and criteria for eligibility. Rula stresses that, to unravel the health maze of today’s world, there needs to be a balance between not only making the number of access points ever-closer to one, but also equipping clinicians in a way that enables them to devote the majority of their time to triage and therapeutic work.

Behavior is shifting: There‘s a growing awareness of the need to switch providers if you aren‘t being heard, requesting specialized care (trauma-informed therapy), using telehealth when availability is tight, and other approaches. But a large proportion also is waiting until depression or anxiety is very severe, often because they believe they should figure it out or fear they can‘t afford it. (“Rula” 2026) Better early warning education and more user-optimized tools that get people to the right level of care more quickly are needed.
